SD Occupational Therapy is an Occupational Therapy service committed to supporting individuals to live safely, independently and confidently in their own homes while improving their overall quality of life. We specialise in working collaboratively with individuals to identify goals and complete home assessments to make recommendations that support them to achieve their goals through the Support at Home Program.

We are committed to delivering evidence-based recommendations that comply with funding guidelines. We endeavour to maintain clear communication with appropriate stakeholders, set realistic goals and provide documentation in a prompt manner to support individuals to remain living at home safely and as independently as possible.

Occupational Therapy helps people live safely and independently in their own homes. An Occupational Therapist looks at how you manage everyday activities and how your home environment works for you, then provides recommendations to make daily life easier and safer to continue ageing in place.

In your first session, your occupational therapist will conduct a thorough assessment to understand your goals, daily activities, challenges and your home environment. They will work with you to discuss recommendations aimed at helping you stay safe and independent at home. This may include education, assistive technology, and/or home modifications.

If assistive technology is recommended, your Occupational Therapist will arrange a trial to ensure the equipment is suitable for your needs. Low-cost items can be purchased directly without a trial.

No, you do not need a referral from a health professional to see an Occupational Therapist through the Support at Home Program. However, a referral form will need to be completed prior to the Occupational Therapy assessment to proceed. A referral can be completed by a participant, their next of kin or authorised representative or a care partner.

We currently do not have a waitlist. We have immediate availability in South East Melbourne and Bundoora/Mill Park area. Once a referral is received, our Occupational Therapist can begin to process this and schedule your assessment!
